Is this the earpiece or the bottom speaker?
It is the bottom loudspeaker, used for ringtones, alarms, media and speakerphone. The earpiece at the top is a separate part.
Why can I hear calls but not my ringtone?
That points to the loudspeaker. The earpiece handles call audio, so a working earpiece and silent ringer usually means this part has failed.
Will iOS show a message after I replace it?
Some iPhone models may show an unknown part or parts history notice in Settings after a component is replaced. The notice is informational and the part continues to work normally.
