Fits the iPhone 12 Pro Max only. It does not fit the iPhone 12, iPhone 12 mini or iPhone 12 Pro, which use their own antenna flex. Check your model in Settings > General > About before ordering.
Is the antenna flex your fault?
Before replacing the cable, update iOS, reset network settings in Settings > General > Transfer or Reset iPhone > Reset, and test near your router. If Wi-Fi and Bluetooth are still weak, or the flex is visibly torn or its connector is lifted after a previous repair, this cable is the likely fix. If Wi-Fi or Bluetooth is greyed out entirely, the fault may be on the logic board instead.
